For non-urgent issues, please follow the guidelines below and choose the right service. An appointment may not be necessary.

How to book an appointment

It may not be necessary for you to attend for an appointment. You can contact us online listing details about your condition or your administration request and you will receive a response from the practice usually within 24 hours but you will be advised of the timeframe once you have submitted.
We will ask that you contact the surgery on the day you need to see a Doctor or Nurse Practitioner and we will endeavour to give you an appointment time to either be seen, or talk to a Doctor or Nurse Practitioner on the phone.
Types of appointments
Routine appointments
These are appointments that can be booked up to 6 weeks in advance. Each appointment is 10 mins long and can be booked at any time of the working day. Please note that there is currently an average wait of 2 weeks for a routine appointment, please plan your appointment to allow for this waiting time.
If your problem is too difficult for 10 minutes you can ask for a double appointment when you book.
Emergency appointments
These appointments are available on the day and are for emergencies that cannot wait until a routine appointment. To book an emergency appointment in the morning session you must call at 8am. To book an afternoon emergency appointment please call at 2pm. Please note that these appointments are not for the signing of forms, non-urgent referrals or repeat prescriptions.
Telephone consultations
These are telephone call-back appointments with a Doctor. These appointments are very limited and so to book a telephone consultation please call at 8am.
Sickness certificates (fit notes)
You must give your employer a doctor's 'fit note' (sometimes called a 'sick note') if you've been ill for more than 7 days in a row and have taken sick leave. This includes non-working days, such as weekends and bank holidays.

Home visits
Whilst we encourage our patients to come to the surgery, where we have the proper equipment and facilities available, we do appreciate this is not always possible. In this respect, if you do need a home visit, you can help us by calling reception before 10:30am.
You may only request a home visit if you are housebound or are too ill to visit the practice. Your GP will only visit you at home if they think that your medical condition requires it and will also decide how urgently a visit is needed. Please bear this in mind and be prepared to provide suitable details to enable the doctor to schedule house calls.
Please note a lack of transport is not considered reasonable grounds for a home visit.
You can also be visited at home by a community nurse if you are referred by your GP. You should also be visited at home by a health visitor if you have recently had a baby or if you are newly registered with a GP and have a child under five years.
Duty doctor service
For when you have an urgent medical problem available 8am to 6.30pm Monday to Friday (excluding bank holidays)
This service offers you quick access to your GP practice, where they know you and have your full medical record. We recommend this service instead of going to A&E at the hospital.
- Phone the surgery and explain to reception that you have an urgent problem
- Reception will ask the nature of your problem and can then put you on the Duty Doctor list
- The Duty Doctor will ring you back within 2 hours
- The Duty Doctor will decide with you what you need, which could be:
- Advice over the phone
- An appointment at the surgery
- A home visit if you are really not well
- A referral to another service
Interpreter
If you would like and interpreter during your consultation please let the reception team know when booking your appointment so this can be arranged.
Change or cancel an appointment
We know people forget things, cancelling GP appointments being one of them. Remember to cancel your appointment if you don't need it.
How to cancel an appointment
If you are unable to keep your appointment, please give us as much notice as possible so that your appointment can be offered to someone else.
If you are cancelling please telephone us on 020 8802 2882 as soon as possible, and at least 30 minutes before your appointment time. This will enable us to offer your appointment to someone else.
Please remember that there may be a shortage of appointments and when you fail to attend you have prevented someone else from being seen in your place. On average about 230 appointments are missed each month
Cancellations notified less than 30 minutes before the appointment time will be recorded as failure to attend (DNA).
Repeated failure to attend booked appointments is a significant waste of NHS resources
NHS e-referral service
The NHS e-Referral Service (e-RS) combines electronic booking with a choice of place, date and time for first hospital or clinic appointments. Patients can choose their initial hospital or clinic appointment, book it in the GP surgery at the point of referral, or later at home on the phone or online.

Out of hours
Enhanced access
We offer early morning appointments (7:30am to 8am) and evening appointments (6:30pm to 7pm). Please ask at reception for more information.
Primary care GP extended access centres
There are now extra GP appointments in the evenings and at weekends available to City and Hackney patients.
Urgent and routine appointments with a GP, either same day or in advance are available at any one of our five primary care GP practices, making it much easier to get the care you need at a location that is convenient for you.
Who can access the service?
Patients registered with a City and Hackney GP practice can book an appointment at the service through their GP practice. These appointments are not suitable for all types of problems. (NB This is not a walk-in service).
What are the opening times?
The opening hours are 4pm to 8pm weekdays and 8am to 8pm weekends and bank holidays.
How do I book an appointment?
For now you book an appointment by contacting your own GP practice however, direct booking is in the pipeline.
What can I expect at a consultation with a GP?
One of the added benefits of the primary care extended access service is that the GP will have access to view your medical record during your visit to the primary care access centre. Having access to view your medical record means that the clinician will have a greater understanding of your medical history and health needs and can then choose the treatment that is right for you. The clinician will ask you for your consent before accessing your medical record.
Using this service will not affect your registration at your usual GP practice.
Who will have access to my medical record?
Once you have consented, only authorised staff within the service will have access to view your medical record. Clinicians can only review your medical record if you have given your expressed consent. If you do not consent then the GP will not be able to continue the consultation. Your medical information will be treated with the strictest of confidentiality. All City and Hackney GP practices have agreed to sign up to a data sharing agreement which sets out what information will be shared, how and when.
What if I don’t want my medical records to be shared?
If you don’t want your medical records to be viewed then you will need to tell the receptionist or GP at your own practice when they are booking the appointment. They will then offer you an appointment at your own GP practice.
Your views and feedback about the primary care extended access service.
At the end of the consultation with the GP you will be asked to complete a patient experience survey. Your views and feedback are important and will help us to make improvements.
Tests and referrals
The GPs in these centres will send any requests for tests and referrals back to your own GP practice to action – in a small number of cases your own GP practice might recommend a different management plan to the extended access GP.

Life threatening
Call 999 or go to A&E now if:
- you or someone you know needs immediate help
- you have seriously harmed yourself – for example, by taking a drug overdose
A mental health emergency should be taken as seriously as a medical emergency.
Urgent but not life threatening
Visit an urgent care centre if:
- You have an urgent medical issue requiring on the day attention
Non-urgent
Use NHS 111 if:
- You need help now, but it’s not an emergency
There will be someone to provide you with advice and to direct you to a clinician if it is necessary.
